The ICS Integrated Care Partnership

The Integrated Care Partnership (ICP) will be a statutory committee within the ICS bringing partners together to develop shared strategies and align ambition. This will include partners from health, social care, public health, the voluntary and community sectors, and the views of people who use health and care services and communities.

The ICP will develop an integrated care strategy to address the health, social care and wellbeing needs of the local population with a particular focus on tackling the wider determinants of health, external factors that impact on someone’s health and wellbeing such as pollution, housing needs and so on.

We are currently developing our ICP in Surrey Heartlands and how it will align with our Health and Wellbeing Board and will update this page as our plans progress.
