NHS England and NHS Improvement have produced a video featuring Surrey Heartlands partner Epsom and St Helier University Hospitals NHS Trust and its Epsom Health and Care ‘@home’ multidisciplinary team initiative.

Epsom Health and Care ‘@home’ champions greater team working between different types of clinicians, social workers and other health and care professionals. The video also features Richard as an example of how it is helping to keep people like him out of hospital.

The service is provided through a partnership between Epsom and St Helier University Hospitals NHS Trust, Surrey County Council which provides social care, community health care provider CSH Surrey and all of the local GP practices.

This way of working is recognised as more rewarding for staff with patients benefiting from high-quality care closer to home. Epsom and St Helier Chief Executive Daniel Elkeles said: “Really proud to see our staff sharing their experience of creating a truly integrated team across the traditional boundaries of acute, primary, social and community care. We have created some amazing partnerships, and this video is definitely worth a watch!”
